A sincere effort is made to see patients promptly and keep waiting time to a minimum. During your first visit to our office, we will establish an office patient record. For your convenience, we have developed a Health and Insurance Questionnaire form that contains the information needed to establish your record. Health and Insurance Questionnaire (PDF) Also at this visit, Dr. Cook will conduct a thorough examination, which includes a blood pressure measurement, an oral cancer screening evaluation, and a complete periodontal examination. Prior to the second visit, Dr. Cook will evaluate the examination findings. Patients will then return to his office for an individual consultation. At this second appointment, the recommended treatment, fees and payment options will be thoroughly discussed and all questions answered. There is no additional fee for this consultation visit. In many cases, it is necessary for Dr. Cook to work collaboratively with other dental specialists and restorative dentists. In these instances, the patient benefits from the combined expertise of two or more doctors. We also have a highly competent and caring staff to help you with all aspects of your treatment, including insurance issues and financial arrangements.
