Dr. Cook believes that he has an obligation to treat each patient as an individual and to listen closely to each patient's questions and concerns.  A sincere effort is made to see patients promptly and keep waiting time to a minimum.

During your first visit to our office, we will establish an office patient record.   For your convenience, we have developed a Health and Insurance Questionnaire form that contains the information needed to establish your record.   You can print the 4 page Health and Insurance Questionnaire form from our website, enter the information requested, and bring the completed form with you to our office.   You can view and print the form using the link below.

Also at this visit, Dr. Cook will conduct a thorough examination, which includes a blood pressure measurement, an oral cancer screening evaluation, and a complete periodontal examination.   Prior to the second visit, Dr. Cook will evaluate the examination findings.   Patients will then return to his office for an individual consultation.   At this second appointment, the recommended treatment, fees and payment options will be thoroughly discussed and all questions answered.   There is no additional fee for this consultation visit.

In many cases, it is necessary for Dr. Cook to work collaboratively with other dental specialists and restorative dentists.   In these instances, the patient benefits from the combined expertise of two or more doctors.
